While all vascular malformations can have an effect on a person’s quality of life to some degree, it is rare for these malformations to become life threatening. Most vascular malformations remain relatively small in nature and/or do not encroach upon the airway or vital organs. One of our latest severe cases involved an 18-year-old boy from Casablanca, Morocco who had a very large, very extensive venous malformation on his facial region that was affecting necessary tasks like eating, sleeping, and breathing. A case like this one requires a great deal of care and expertise, time, and the specific knowledge of how these genetic abnormalities grow and develop.
As in this severe case, it is often only a matter of time before one or more of the vessels in the malformation will rupture, resulting in a huge, massive hemorrhage which can be fatal due to extreme blood loss.
Dr. Milton Waner, M.D. BCh, FCS (SA), is the director of the Vascular Birthmarks Institute of New York, and he is considered a world authority in hemangiomas and vascular malformations. Working with his interdisciplinary team, they have been able to treat and thus save many patients from across the world. As with the young man from Morocco, his huge venous malformation was encroaching on his airway so that at some point he would have lost the ability to breathe. Also, this young man’s malformation was so incredibly large that it had completely transformed the entire left side and most of the front side of his face so that he had no distinguishable features other than the malformation.
Dr. Waner and his team faced the extreme delicacy of the procedure to lessen and remove much of this malformation. If there was a rupture of the dilated vessels, it would have led to uncontrollable bleeding, causing the patient to lose up to a liter of blood in a single minute. Although there were moments of difficulty, Dr. Waner and his team were able to remove nearly all of the venous malformation on the patient’s face. His life is no longer in danger, and the malformation is no longer life threatening.
